Where would I find the application
forms and the criteria?
Application forms have been placed at these locations:
City Hall, Community Centers, Colleges, Universities, High
Schools, Bill Copeland Sports Centre, Burnaby Now office,
Burnaby Lake Sports Complex, local Fire Halls. Also, forms
are in the process of being distributed to various local
sports organizations (soccer, baseball, lacrosse, etc.)

Should I provide photographs, and press clippings of
my nominee?
Yes, any verification material is very helpful in the
committee's selection process. Please send in duplicates
-NOT ORIGINALS. If you wish your items to be
returned, a Self Addressed Stamped Envelope
must accompany the submission Otherwise, all
submission documents and forms will become the property of
the Burnaby Sports Hall of Fame.
Is it necessary for the athlete, team, coach, or
builder to be active in the current year?
No, the committee will evaluate individuals and teams
from Burnaby's long athletic history. Inductees can be from
all previous years.
When should my application be submitted to the
committee?
On or before Nov. 18, Friday,
2011. The committee will review the selections after that date.

Can I email the application?
Sorry, only hard copy nomination forms will be
accepted. Fax submissions are not accepted. Please mail
the submission or bring it into the Parks and Recreation
office (address listed
above)

Where can I order tickets? Banquet
date: Thurdsay, February 16, 2012.
Tickets for banquet: $75
each, and $45 for past
inductees.
